Making a U-PLUS 24 Hour Accident claim (Non-Work Related)

To ensure your claim is processed promptly and to avoid any unnecessary delays please ensure you have provided Coverforce with all documentation described in the following checklist:

 

»  Group Risk Employer's Form
Completed by Employer

»  Group Risk Member's Form
Completed by Employee

»  Group Risk Attending Doctor's Statement
Completed by the Treating Doctor

»  Wage records for the 26 week prior to the accident (gross)
Please provide week to week listing showing gross payable, allowances etc.

»  Any copies of medical reports or hospital discharge reports etc.
Please note: These are only required if they are in your possession

»  Two or more Witness Reports Completed on Statutory declarations

»  A certified photocopy of photographic identification
Eg. Current Drivers License or Current Passport

»  Tax File Number Declaration (Australian Taxation Form)
