To ensure your claim is processed promptly and to avoid any unnecessary delays please ensure you have provided Coverforce with all documentation described in the following checklist:
- Group Risk Employer's Form
Completed by Employer
- Group Risk Member's Form
Completed by Employee
- Group Risk Attending Doctor's Statement
Completed by the Treating Doctor
- Wage records for the 26 week prior to the accident (gross)
Please provide week to week listing showing gross payable, allowances etc.
- Any copies of medical reports or hospital discharge reports etc.
Please note: These are only required if they are in your possession
- Two or more Witness Reports Completed on Statutory declarations
- A certified photocopy of photographic identification
Eg. Current Drivers License or Current Passport
- Tax File Number Declaration (Australian Taxation Form)
