Making a U-PLUS 24 Hour Sickness claim (Work Related)

To ensure your claim is processed promptly and that you do not incur any unnecessary delays please ensure you have provided Coverforce with all documentation described in the following checklist:

 

»  Employers Statement
Completed by Employer

»  Employee Preliminary Claim Form
Completed by Employee

»  Preliminary Medical Attendants Statement
Completed by the Treating Doctor

»  Confirmation of Gross Weekly Workers Comp. rate employee has been receiving since date of diagnosis
Please provide a copy of the correspondence received from the workers compensation insurers advising that
liability has been accepted and stating the gross weekly workers compensation rate payable

»  Wage records for the 26 week prior to the date diagnosis (gross)
Please provide week to week listing showing gross payable, allowances etc.

»  Any copies of medical reports or hospital discharge reports etc.
Please note: These are only required if they are in your possession

»  A certified photocopy of photographic identification
Eg. Current Drivers License or Current Passport

»  Tax File Number Declaration (Australian Taxation Form)
