UPlus Claims

Through UPlus, you can make claims directly through our team.

In order for your claim to be processed, please kindly provide us with:

  • Proof to our satisfaction of any event or condition establishing your entitlement or any insured persons entitlement to benefits under the policy.
  • Please note that benefits are payable in accordance with the terms of the policy.

For your construction income protection claims to be valid, you must notify us immediately in writing of:

  • Any accident, sickness or injury suffered by any Insured Person lasting for more than 30 days,
  • Any other circumstances which may lead to a claim under the Policy.

You and the insured person must comply with our claim requirements. All information requested should be provided to U-Plus Pty Ltd who will pass onto us for the purpose of assessing the claim. The information we require includes:

  • Completed claims forms,
  • Medical evidence and reports from treating doctors (we are entitled to have the insured person examined by a medical practitioner or other qualified person nominated by us at our expense),
  • UPlus employer statement,
  • Tax file number declaration form (issued by ATO),
  • Information or records relating to the income, attendance, and duties of the insured person,
  • A copy of your Job Description,
  • Certified copy of the death certificate of the insured person (if applicable),
  • Any other information we deem necessary for the purpose of assessing the claim.

Making a claim

To ensure your claim is processed promptly and to avoid any unnecessary delays please ensure you have provided Coverforce with all documentation described in the following checklist:

  • Section A of Claim Form – Insured Person’s Statement
    Completed by Employee
  • Section B of Claim Form – Medical Practitioner’s Statement
    Completed by the Treating Medical Practitioner
  • Section C of Claim Form – Employer’s Statement
    Completed by Employer
  • Confirmation of Gross Weekly Workers Compensation rate employee has been receiving since date of injury
    Please provide a copy of the correspondence received from the workers compensation insurers advising that liability has been accepted and stating the gross weekly workers compensation rate payable
  • Wage records for the 26 weeks prior to the accident (gross)
    Please provide week to week listing showing gross payable, allowances etc.
  • Copies of all WorkCover Medical Certificates
    Please note: These are only required if they are in your possession
  • Site accident report
    Should a site accident report not be available two or more witness reports completed on statutory declarations and signed by a Justice of the Peace are required.
  • Tax File Number Declaration (Australian Taxation Form)

  • Section A of Claim Form – Insured Person’s Statement
    Completed by Employee
  • Section B of Claim Form – Medical Practitioner’s Statement
    Completed by the Treating Medical Practitioner
  • Wage records for the 26 weeks prior to the accident (gross)
    Please provide week to week listing showing gross payable, allowances etc.
  • Copies of all Medical Certificates, medical report and hospital discharge summaries
    Please note: These are only required if they are in your possession
  • Tax File Number Declaration (Australian Taxation Form)

    Completed by Employee
  • Medical reports to support your claim
    Eg. Treating doctors report and Workcover Independent Medical Report
  • Certificate of Determination
    Complying Agreement or an Application to Resolve a Dispute
  • Proof of Payment
    A copy of the cheque or remittance advice from your workers compensation insurers payment
